We believe it is a blessing to remember the joy and kindness that loved ones who have passed away brought to our lives. While honoring someone important to you, you can make a significant difference in the lives of our neighbors nearing the end of life, as well as their families. The cost to add a name to the CarePartners Memorial Wall is a one-time contribution of $250, with proceeds supporting CarePartners patients in need.
Memorial and Tribute Gifts
A gift in memory of a loved one or in honor of a friend or family member is a thoughtful way to express your thanks and support.
If you would like to memorialize someone with a gift to CarePartners, we will notify their spouse, children, or other loved one of your special gift. Please provide the name and address of the person to be notified. The amount of your gift will remain confidential.
If you would like to honor a family member, friend, or CarePartners staff, we will send the honoree a letter to let him or her know of your gift. The amount of your gift will remain confidential.
Holiday Ornaments are Coming Soon!
Each year, CarePartners Foundation offers handmade, limited-edition memorial ornaments. The 2017 ornament is a dove made by Mud Dabbers Pottery in Brevard. Ornaments are a lovely way to honor or remember another while also supporting hospice care.
CarePartners Hospice Solace Center
You can memorialize a loved one through naming opportunities at the CarePartners Hospice John F. Keever, Jr. Solace Center. Opportunities include trees and garden areas, rooms, benches, and our donor recognition wall. We are no longer engraving bricks. CarePartners PACE (Program of All-inclusive Care for the Elderly) is a program for people age 55 or older that need support to remain safely at home. It provides community-based care and services for people who might otherwise need a nursing home level of care. Click to learn more about PACE.

While cash donations are usually the most effective way to help us provide for our patients’ needs, there are certain times when gifts-in-kind can be useful. Gifts-in-kind are most frequently needed for our Equipment Loan Program and for fund-raising events. To find out what items would be useful, please contact us.
